Cancellation and Refund Policy

This policy describes the cancellation and refund terms for services offered by Blue Oak Services LLC dba Prescriva, a California limited liability company. This policy is part of and subject to our Terms and Conditions.
Overview
We want you to be satisfied with your experience at Prescriva. This policy outlines the circumstances under which refunds and cancellations are processed. By using our Services, you agree to the terms of this policy.
No specific results are guaranteed by enrollment in our Services. Individual outcomes vary based on adherence to prescribed treatment, diet, exercise, and overall health.
What Your Subscription Covers
Your Prescriva subscription begins after you complete an intake assessment, at which point your first charge occurs. Your subscription includes access to the following services (coverage may vary based on your plan):
- Access to licensed healthcare providers within our Provider Network
- Provider consultations and clinical reviews
- Pharmacy fulfillment and medication coordination
- Ongoing clinical monitoring and follow-up
- Online patient support
Refund Eligibility
You are entitled to a refund in the following circumstances:
- Medical disqualification: If a licensed healthcare provider determines that you are not a candidate for treatment after completing your health assessment, you will receive a refund of your most recent billing cycle. Refunds for medical disqualification apply only to the current billing cycle; prior billing cycles are not eligible for refund.
- Medication out of stock: If the prescribed medication is unavailable from our pharmacy partners and no suitable alternative can be provided, you will receive a full refund.
- Service not delivered: If you have been charged but your assessment has not been reviewed by a healthcare provider within the stated timeframe, you may request a full refund.
Non-Refundable Situations
Except as described in the Refund Eligibility section above, no refunds will be issued upon cancellation. Refunds are not available in the following situations:
- Prescription medications: Under federal law, prescription medications cannot be returned once dispensed. Medications that have been compounded and shipped are non-refundable and non-returnable per pharmacy regulations and applicable federal and state law.
- Completed consultations: Fees for medical consultations are non-refundable once a provider consultation has been completed.
- Partial billing cycle: No partial refunds are provided for unused portions of a subscription billing cycle after cancellation.
- Change of mind: Refunds are not provided if you decide you no longer want the medication after it has been prescribed and dispensed.
- Prior billing cycles: Refunds apply only to the current or most recent billing cycle. Prior billing cycles are not eligible for refund.
Damaged or Incorrect Medications
Please inspect your medication shipment promptly upon receipt. If you receive a damaged or incorrect medication:
- Damaged medications: Contact us immediately at help@prescriva.com with photographic evidence of the damage. We will coordinate with our pharmacy partner to send a replacement at no additional cost.
- Incorrect medications: If you receive a medication that does not match your prescription, contact us immediately. The pharmacy will send the correct medication. Do not use any medication that does not match your prescription.
Subscription Cancellation
Our services are offered on a subscription basis. You may cancel your subscription at any time through:
- Your patient dashboard account settings
- Contacting us at help@prescriva.com
Cancellation Notice
To ensure cancellation is processed before your next billing cycle, please submit your cancellation request at least 72 hours prior to your next billing date. If your cancellation request is received less than 72 hours before your billing date, the next billing cycle may still be charged, and your cancellation will take effect the following cycle.
When Cancellation Takes Effect
Your cancellation will take effect at the end of your current billing cycle. You will continue to have access to services through the remainder of the paid period. No further charges will be made after your cancellation takes effect.
Reactivation
You may reactivate your subscription at any time. A new health assessment may be required depending on how long your account has been inactive.
How to Request a Refund

Please include in your request:
- Your full name and email address associated with your account
- The reason for your refund request
- Your order or subscription reference number (if available)
Processing time:Refund requests are reviewed within 5 business days. Approved refunds are processed back to the original payment method and may take 5–10 business days to appear on your statement, depending on your financial institution.
Case-by-Case Refunds
Refund requests for circumstances not described above will be evaluated on a case-by-case basis. We strive to be fair and reasonable in all refund decisions. Contact help@prescriva.com to discuss your situation.
Changes to This Policy
We may update this Cancellation and Refund Policy from time to time. We will notify you of material changes by posting the updated policy on our Platform with a new effective date. The policy in effect at the time of your purchase or most recent subscription renewal will apply to that transaction.
